چکیده

In this work discrete event simulation models developed in Witness are discussed and used to evaluate performance indices of interest related to the scheduling of operating theatre activities for surgical departments; in particular the attention is focused on the wards’ productivity in terms of utilization rate, throughput and overruns. The first model reproduces the behaviour of the operating theatre and tunes the parameters. Subsequently, alternative operative scenarios are proposed by comparing the actual time table and schedule with that obtained by using a blocked booking criterion for the weekly scheduling of the surgery rooms. The sequencings of the surgical activities are analysed according to different priority rules, derived from the classical scheduling theory. In the analysis the possibility of introducing the so-called “pre” and “post” recovery room is considered with respect to the activity level of the department. The computational results presented in the paper are related to a one-year simulation horizon for the surgical department of a hospital located in Genoa, Italy.

Operating Room Scheduling in Teaching Hospitals: A Novel Stochastic Optimization Model

Background and Objectives: Operating room (OR) scheduling is key to optimal operating room productivity. The significant uncertainty associated with surgery duration renders scheduling of surgical operation a challenging task. This paper proposes a novel computational stochastic model to optimize scheduling of surgeries with uncertain durations. The model considers various surgical operation co...

Consequences of running more operating theatres than anaesthetists to staff them: a stochastic simulation study.

BACKGROUND Numerous hospitals implement a ratio of one anaesthetist supervising non-medically-qualified anaesthetist practitioners in two or more operating theatres. However, the risk of requiring anaesthetists simultaneously in several theatres due to concurrent critical periods has not been evaluated. It was examined in this simulation study. METHODS Using a Monte Carlo stochastic simulatio...

Investigating Pareto Front Extreme Policies Using Semi-distributed Simulation Model for Great Karun River Basin

This study aims to investigate the different management policies of multi-reservoir systems and their impact on the demand supply and hydropower generation in Great Karun River basin. For this purpose, the semi-distributed simulation-optimization  model of the Great Karun River basin is developed. Also, the multi-objective particle swarm optimization algorithm is applied to optimize the develop...

Patient mix optimisation and stochastic resource requirements: a case study in cardiothoracic surgery planning.

Cardiothoracic surgery planning involves different resources such as operating theatre time, beds, IC beds and nursing staff. In the daily practice of the Thorax Centre case study setting, the planning focuses on optimal use of operating theatre time, though the performance of the Thorax Centre as a whole is often more limited by other resources. For operating theatres a master surgical schedul...

Reducing Operating Costs with Optimal Maintenance Scheduling

The potential benefits of optimal service scheduling for cleaning heat exchangers based on minimizing operating costs are explored. A demonstration is performed on a simulated 20 ton air-to-air vapor compression air conditioner. The costs of regular service schedules (indicative of current practice), minimal schedules needed to maintain comfort, and the optimal schedules are compared. Generally...
